Admissions & Enrollment

Forever Young Adult Center is committed to serving adults (age 18 and older) with a wide range of physical, cognitive, medical, and social needs. We welcome individuals with varying levels of functional limitation, including but not limited to:

• Mild to profound cognitive impairment (dementia, Alzheimer’s disease, traumatic brain injury, intellectual/developmental disabilities, etc.)  
• Physical disabilities requiring assistance with mobility, transfers, or activities of daily living  
• Chronic health conditions (diabetes, heart disease, COPD, Parkinson’s, stroke recovery, etc.)  
• Mental health conditions (depression, anxiety, schizophrenia, bipolar disorder, etc.) when stable and managed  
• Post-hospital rehabilitation needs  
• Social isolation or need for structured daily activities and supervision

We are able to accommodate participants who require:

• Supervision for safety and wandering prevention  
• Assistance with ambulation, transfers, and wheelchair management  
• Toileting assistance (including incontinence care and scheduled prompting)  
• Medication reminders or administration (by qualified staff)  
• Assistance with eating/feeding  
• Behavioral support and redirection  
• Specialized activities for different cognitive and functional levels

Admission is determined on an individual basis through a pre-admission assessment conducted by our clinical team. We reserve the right to decline or defer admission if, after thorough assessment, we determine that the individual’s needs exceed the scope of service our program can safely and effectively provide (e.g., 1:1 nursing care 100% of the time, active intravenous therapy, ventilator dependence, or behaviors requiring constant physical restraint).

Because we individualize care, our private-pay rates are tiered according to the level of assistance and supervision required. During the pre-admission assessment, we will determine the appropriate care tier and provide a clear written cost.
